LOUISVILLE, KY (WAVE) – A daycare has closed all its facilities two days after a deadly crash.

The Cabinet for Health and Family Services released information stating the owner of Heavenly Angels, Lavonia Lewars, notified the Cabinet's Office of the Inspector General of her intent to close all three of her facilities.

Tuesday, the driver of one of the daycare's vans lost control and slammed into a tree. A Heavenly Angels Childcare worker in the van was killed.

The Kentucky Cabinet for Health and Family Services said in a statement, "the owner of Heavenly Angels has notified the Cabinet's Office of the Inspector General of her intent to voluntarily close her three facilities. The Cabinet has confirmed that Heavenly Angels is not currently caring for children."

Reached by phone earlier this week, daycare owner Lavonia Lewars said, "I'm just praying for my kids and the one that I lost, and focusing on comforting the kids and their families at the hospital."

A WAVE 3 Troubleshooter Investigation uncovered Heavenly Angels Childcare vans had been cited for safety violations long before the deadly accident.

Lewars has made her decision to shut down Heavenly Angels voluntarily. The daycare is not currently caring for children.
