On May 6, 2013, 33-year-old Jorge Espinoza was headed east on Interstate 8 in Yuma in a 2013 Volvo tanker-truck.
Video from his dash cam captures the moment Espinoza is seen ignoring a lane closure and slammed into two Department of Public Safety patrol cars and two fire department trucks.
The crash killed DPS officer Tim Huffman, who was sitting in his cruiser writing a report.
Through a public records request, CBS 5 obtained the 607-page investigative report detailing the crash.
